29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees operating forklifts engaged in changing propane tanks were exposed to fire and explosion or projectile hazards in that compressed gas cylinders, that were not in use, were not secured to prevent damage in accordance with Compressed Gas Association Pamphlet P-1-1965. Three propane gas cylinders located outside, near the propane storage rack, near bagging, on the west side of the building.

29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD) At the bagging, crushing and mixing areas,employees engaged in fertilizer manufacturing activities while voluntarily using respirators were exposed to health hazards in that the employer had not provided the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, to employees. Employees are provided and voluntarily use 3M Half Mask Respirators and N95 respirators on site.

In the crushing and bagging areas, the employer failed to develop a written hazard communication program to describe how the program would be implemented to include proper training, labeling, and the administration of safety data sheets, to address their chemicals such as the hazards of White Dolomite and Brown Dolomite which are corrosive and may cause skin and eye irritation; Gypsum, which is a respiratory hazard and eye and skin irritant.
