Generator Room - On or about September 28, 2022, the employer failed to ensure that the authorized employee performing service and maintenance on a motor mounted on the side of an exhaust fan, model 36AFBV-C, serial no. 006SC21859-00/0000701, locked out the machine. The authorized employee was exposed to unexpected energizing, start up, or release of stored energy, when another employee locked out the machine.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices were temporarily removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): (a) Generator Room - On or about September 28, 2022, the employer failed to ensure that employees were removed from the machine when a lockout device was temporarily removed to test the motor's belt and pulley. An employee sustained a left index fingertip amputation when the lockout device was removed to test the motor.
