29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms:  Location: 120 Main St. Mount Kisco, NY 10549   a) On or about 10/12/2022, employees were exposed to fall hazards while painting windows on a supported scaffold approximately 21ft without a guardrail system along all open sides exposing employees to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location: 120 Main St. Mount Kisco, NY 10549   a) On or about October 12, 2022 an employee was conducting window painting work on a commercial building approximately 21ft using a ladder without an extension of at least 3ft to get on and off the landing surface of a scaffold.
