29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  (a) Front Side, West End of Two-Story Section of the Building - The competent person did not inspect and ensure the correct use of all components of personal fall protection equipment as evidenced by the fact that an employee used a 50-foot long Guardian vertical lifeline rope, with fraying at the shock pack end, to tie off to the basket of a JLG aerial lift, and the employee did not adjust the rope through the rope grab to a length that would restrain the employee from falling from the basket nor to a length to prevent the employee from free falling more than 6 feet, on or about October 11, 2022.  No abatement certification or documentation required.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.)   (a) Front Side, West End of Two-Story Section of the Building - An employee wore a harness and used a 50-foot long Guardian vertical lifeline rope  to tie off to the basket of a JLG aerial lift while the employee worked at the building's second story high eave.  The employee did not adjust the 50-foot long vertical lifeline rope to a length that would restrain the employee from falling from the basket nor to a length to prevent the employee from free falling more than 6 feet.  In addition, the vertical lifeline rope had frayed at the shock pack end, on or about October 11, 2022.  No abatement certification or documentation required.

29 CFR  1926.454(a)(1): The employer failed to have each employee who performed work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area:   (a) Front Side, West End of Two-Story Section of the Building - Each employee who worked from an aerial lift did not receive training in appropriate fall protection methods to avoid fall hazards associated with the work, on or about October 11, 2022.  No abatement certification or documentation required.
