29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) 603 Lauschtown Road, Denver, PA, On or about October 26, 2022, the employer did not institute a continuing and effective hearing conservation program when employees' noise exposure exceeded an 8 hour time weighted average sound level of 85 dBA.  A) An employee conducting molding/folding machine operations was exposed to noise at a level of 83.1% of the allowable limit during a 456 minute sampling period on October 26, 2022; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 88.6 dBA for the dose and time of the employee exposure.  B) An employee conducting foundry operations was exposed to noise at a level of 54.6% of the allowable limit during a 393 minute sampling period on October 26, 2022; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 85.6 dBA for the dose and time of the employee exposure.  C) An employee conducting molding operations was exposed to noise at a level of 136.7% of the allowable limit during a 389 minute sampling period on October 26, 2022; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 92.2 dBA for the dose and time of the employee exposure.  D) An employee conducting processor operations was exposed to noise at a level of 104.27% of the allowable limit during a 457 minute sampling period on October 26, 2022; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 90.3 dBA for the dose and time of the employee exposure.  E) An employee conducting processor operations was exposed to noise at a level of 126.5% of the allowable limit during a 386 minute sampling period on October 26, 2022; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 91.6 dBA for the dose and time of the employee exposure.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
