29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) Front Entrance, Second Story Roof Structure - The employer had no competent person on the job site in that no employees had any authority. As a result, nobody inspected the job site, identified predictable fall hazards, and took immediate actions to correct the hazards, on or about October 11, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. (a) Front Entrance, Second Story Roof Structure - Employees did not tie off while detailing steel roof trusses on the roof structure, exposing the employees to falling 15 feet 4 inches from the roof level to the second floor (interior fall) and to falling two stories from the roof to grade (exterior fall), on or about October 11, 2022. No abatement certification or documentation required.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Front Entrance, Second Story Roof Structure - Employees detailing steel roof trusses did not have proper access to the roof structure. The employees used a 12-foot step ladder set up on the second floor to access the roof structure, 15 feet 4 inches above the second floor level, exposing employees to falling to lower level, on or about October 11, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
