29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: Outdoor parking area outside of building entrance: The long extension cord used to provide power from a gasoline powered generator to a rotary hammer exhibited signs of overheating ("pig tailing"). Employees were exposed to electrical and fire hazards.
