29 CFR 1928.57(a)(6)(iv): Employer did not ensure everyone was clear of machinery before starting the engine, engaging power, or operating the machine: Oshkosh Heifer Development, LLC - Miller Circle - Dorman Property -16 miles north of Oshkosh, Nebraska - Employer did not ensure that employees were not in the hazard area of operating machinery. On or about October 8, 2022, an employee scooping spilled corn into the running combine head was caught by the gathering chain and suffered a double leg amputation.
