29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) At 147 Fritz Mar Lane, Athens, GA 30607, Production Area; On or about September 28, 2022, and at times prior to, the employer exposed employees to amputation hazards while operating the automatic record reclaimer "dinker", in that a guard at the point of operation was not provided to employees operating this equipment.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) At 147 Fritz Mar Lane, Athens, GA 30607, Production Area; On or about September 28, 2022, an employee operating an automatic vinyl record reclaimer "dinker" suffered an amputation below the right proximal index finger while attempting to clear a jam. The employer failed to report the work-related amputation incident within 24-hours.
