29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: At a work site located at 301 Carmeco Road, Lebanon, Missouri 65536, employees were exposed to caught between and crushed by hazards while conducting forklift operations. Employees used and had access to the Yale forklift with missing markings on the operators control levers while moving materials throughout the facility.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): At a worksite located at 301 Carmeco Road, Lebanon, Missouri 65536, an employee was exposed to caught between and crushed by hazards while conducting metal stamping operations. An employee was feeding rolled sheet steel into a Boss metal press by remote control with one hand while placing the index finger from the other hand at the point of operation without appropriate guarding in place.
