29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system. Location: 17c West Taylor Steet, 102 Hampton, VA. On or about: 13 October 2022. a) Employees worked to configure the flow collector; employees climb a ladder to access hoist blocks, crossing the entry lip of the tunnel. The ladder's platform has an entry point that is open and not effectively guarded.

At the top of the ladder there is an open gear box for rollup doors not effectively guarded.
