29 CFR  1910.95(g)(6): "Annual audiogram." At least annually after obtaining the baseline audiogram, the employer shall obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.  a.) The employer did not ensure an audiometric testing program was established and maintained by making annual audiometric testing available for employees exposed to noise above the action level of 85 decibels while grinding brazing pans in that, noise monitoring was conducted on October 27, 2022, and sample results revealed employees were exposed to noise at 90.3 decibels while grinding brazing pans for 8 hours.

Information provided in the training program shall be updated to be consistent with changes in protective equipment and work processes.  a.) 29 CFR  1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  a.) On or about October 11, 2022, employees welding and grinding baffles and brazing pans were exposed to hazardous chemicals such as but, not limited to Nucor carbon and alloy steels (carcinogen) and Ryerson stainless steel (carcinogen, flammable) and did not receive training on the composition and health hazards of the metals; and did not know the location of the safety data sheets.
