1926.503(a)(2) The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: a) On or about October 14, 2022, employees conducting wood truss installation on a structure, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 22 feet, onto grass and gravel.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) On or about October 14, 2022, employees constructing a building were not provided a ladder for access to the upper portion of structure where work was being performed. This exposed employee(s) to a fall hazard of up to twenty-one (21) feet while climbing the framework of the structure.
