29 CFR 1926.451(b)(1)(i):Each platform unit was not installed so that the space between the platform and the unit was no more than 1 inch wide. On or about October 13, 2022, four employees were working on a scaffold that was 63 feet in height in which the working area of the top scaffold had open gaps on the platform, exposing the employees to fall hazards.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about October 13, 2022, four employees were working on the top of the concrete structure without using fall protection up to approximately 63 feet high, exposing the employees to fall hazards.
