On or about October 11, 2022, at the above addressed jobsite, an employee cleaning the building interior during construction activities was throwing material from a wall opening into a job made box attached to a forklift, approximately fourteen (14) feet two (2) inches above ground. The employee was not protected from falls, thereby exposing the employee to fall hazards.
