29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. DS Services of America, Inc., Watertown, Connecticut, on or about 10/11/2022 - An employee was assigned to load trucks operating a forklift. The employee was not certified in writing for the operation of the forklift. DS Services of America, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number 1401279, citation number 1, item number 2/a, and was affirmed as a final order on 08/22/2019, with respect to a workplace located at 2300 Windy Ridge, ATLANTA, GA 30339.
