Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about October 07, 2022, an employee of the subcontractor performing sight glass installation at the tank storage area parked a motorized vehicle in close proximity to a flow back tank.
