29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. Location: 431 2nd Street, Everett, MA On or about 10/13/2022, the energy control program for the Hammel metal shredder was inadequate and did not include work-specific information such as but not limited to, repair tasks or machine conditions which require lockout/tagout, how/when to utilize the lockout/tagout procedures, how training is to be administered for affected/authorized employees, and a description of when periodic inspections of the energy control program would occur.
