29 CFR 1910.28(b)(8): Repair pits, service pits and assembly pits less than 10 feet in depth. The employer did not implement paragraphs (b)(8)(i) - (8)(iii) or require fall protection systems for a repair pit, service pit or assembly pit that is less than 10 feet (3 m) deep. Truck Maintenance Repair Shop - On or about October 17, 2022, and times prior to, the truck repair pit was not adequately guarded when not in use, thus exposing employees to a fall hazard of approximately 5 feet. The employer did not protect their employees from fall hazards by complying with steps outlined in 1910.28(b)(8)(i) through 1910.28(b)(8)(iii).

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1m) or by a noncombustible barrier at least 5 feet (1.5m) high having a fire resistance rating of at least one-half hour: On October 17, 2022, inside the truck maintenance shop, AC Trucking, Inc. located at 133 TR Hughes Boulevard, O'Fallon, Missouri, stored oxygen and acetylene cylinders together. The cylinders were not separated by a distance of 20 feet or a wall 5 feet high with a half hour fire rating.
