On or about 10/17/2022, worksite at 17 Deer Park Rd. Kings Point, NY Employees working on the ground were exposed to head injury from falling objects such as, but not limited to tools and materials from other employees who were working overhead. Protective helmets were not worn.

On or about 10/17/22, worksite at 17 Deer Park Road. Kings Point, NY Employees working on a fabricated frame scaffold were exposed to the hazard of falling approximately 18 ft. to the ground. The working platforms were installed with only two scaffold planks.
