Abatement certification required within 10 days after abatement date. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) TwinMed - Home Care - On or about October 17, 2022, employees were utilizing relocatable power taps (power strips), to power fixed industrial cooling fans, exposing employees to fire and electrical hazard. In addition, the relocatable power taps were not connected directly to an outlet and were being daisy chained between multiple extension cords. The electrical equipment was not used in accordance with the listing. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement. For Reference: Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL White Book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) TwinMed - Home Care - On or about October 17, 2022, extension cords were being utilized by employees to power 110 V fixed cooling fans, as well as relocatable power taps, exposing employees to fire and electrical hazards. b) TwinMed - Shipping Dock - On or about October 17, 2022, an extension cord was being utilized by employees to power a relocatable power tap and office equipment at the workstation between docks #6 and #7, exposing employees to fire and electrical hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a) TwinMed - Dock #30 - On or about October 17, 2022, the exit access of the Exit Route was obstructed by a parked floor scrubber and lift truck. The Exit Route was not kept free and unobstructed as required by this section. TwinMed, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1461436, citation number 1, item number 1, and was affirmed as a final order on March 17, 2020, with respect to a workplace located at 6356 N. Howard St., Niles, No abatement certification or documentation required.
