29 CFR 1910.213(h)(1):The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed. In the C&M shop, the DeWalt Power Shop radial arm saw, model no. 500268, serial no. 62070392, did not have a lower blade guard during the walk-around portion of the inspection. Employees are exposed to an amputation hazard.

29 CFR 1910.334(a)(2)(ii):When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made. Located in the south fork C&M shop, a powered Ridgid-brand pipe tool had evidence of damage on the cord. The cord had been taped with electrical tape and damage to the insulation was visible at the end of the tape as well. Employees are exposed to a shock hazard.
