29 CFR 1904.41(a)(2): An establishment that had more than 20 and fewer than 250 employees at any time during the previous calendar year and the establishment is classified in an industry located in Appendix A, did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee, no later than the date listed in paragraph (c) of this section: Graphite Die Mold dba Morgan Advance Materials, 18 Airline Drive, Durham, CT: On or about April 12, 2023, the employer, having employed 63 people during calendar year 2022 and operating under NAICS code 335991 had not electronically submitted information from their OSHA Form 300A or equivalent to the OSHA Injury Tracking Application (ITA) Database.
