29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite 1531 Sawmill Run Boulevard, Pittsburgh, PA: On or about October 18, 2022, during sign installation activities, the employer did not ensure that employees received appropriate scaffold training. Deficiencies noted during the inspection included one employee in the basket of a bucket truck working at a height of approximately 10 feet while using a shock absorbing lanyard as fall protection and a different employee in the basket while traversing to and from the roof with no fall protection of any kind in use.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that would enable each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite, 1531 Sawmill Run Boulevard, Pittsburgh, PA: On or about October 18, 2022, during sign installation activities, the employer did not ensure that an employee received appropriate fall protection training. An employee worked on top of a roof with no fall protection in use where fall hazards of up to approximately 12 feet existed.
