29 CFR 1910.22(a)(2): The employer did not ensure the floor of each workroom was maintained in a clean and, to the extent feasible, in a dry condition. When wet processes were used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided: Location : Cafeteria Kitchen at Richards Elementary School Instance A: On or about October 5, 2022, employees were exposed to injuries related to slip and fall hazards where employees were observed walking on wet floors created from poor drainage from the Market Forge steamer. When the steamer was in use water poured out of the drain and onto to the kitchen floor creating slippery and hazardous conditions. Instance B: The employer did not to provide sufficient dry standing places such as mats for the employees to walk and work which created additional exposure injuries related to slips and fall hazards.

29 CFR 1910.132(b): Employee-owned equipment: Where employees provided their own protective equipment, the employer did not assure its adequacy, including proper maintenance, and sanitation of such equipment: Location: Cafeteria kitchen at Richards Elementary School On or about October 5, 2022, the employer did not assure that employees who were working on slippery floors were utilizing adequate and proper PPE such as non-slip shoes. Employees were required to provide their own protective shoes however the employer did not assure that this PPE was adequate such that the safety shoes utilized were not slip resistant.
