29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Location: 24 North Front St. New Bedford, MA 02740 On or about 10/19/2022, employees working on the building's roof exterior were exposed to fall hazards of 25 feet, or more, to the concrete floor below. The employer did not supply a guardrail system or personal fall arrest system for employee protection. On or about 10/19/2022, employees working for Franklin Analytical Services, Inc. were conducting demolition of the building. The employer did not conduct an engineering survey to determine structural integrity prior to employees accessing the roof. An employee was working on the roof of the large center building located on the North section, Lot: 79-4 was pushing a wheelbarrow with debris. This employee stepped on a section of the roof and fell 25 feet to the concrete floor below, sustaining serious injuries.

29 CFR 1926.850(a):Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed. Location: Large center building North section Lot. 79-4 @ 26 North Front St. New Bedford, MA On or about 10/19/2022, employees working for Franklin Analytical Services, Inc. were conducting demolition of the building.
