29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:   (a)	On October 12, 2022, the employer did not determine the exact location of an active gas utility pipe that was located below grade prior to using a Case CX3500 excavator to dig an excavation to connect an 8-inch water line.  The operator struck an underground gas line that resulted in a release of natural gas.  This condition exposed employees to burns and other fire hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  (a)	On or before October 19, 2022, the employer did not ensure that employees working inside an excavation that measured approximately 20 feet in length, 12 feet wide, and 7.5 feet in depth were protected by an adequate protective system.  The excavation had near vertical walls, a fissure near the bottom, and the spoils pile was less than two feet from the edge of the excavation.  Employees used a shovel at the bottom of the excavation to dig around the end of a water pipe.  This condition exposed employees to a crushing hazard by excavation wall cave-in.
