(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: in the areas identified in 1910.28(b)(1)(i)(A) - 1910.28(b)(1)(i)(C). On or about September 28, 2022, at location 741 4th St. Menasha Wisconsin: the employer did not ensure an employee engaged in the re-web process for the Manzoni Press was protected from falling off an installed platform on top of the machine, exposing the employee to a fall height of approximately 7 feet 10 inches.

(1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. On or about October 19, 2022, at location 741 4th St. Menasha Wisconsin: employees were not trained on the recognition of fall hazards or the use of fall protection when working on top of the Manzoni Press with a fall hazard of approximately seven feet 10 inches.
