29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 18, 2022 and a week prior, employees working in both the Eastbound and Westbound lanes of Highway 34 on the Great River Bridge were exposed to struck-by hazards when installing, removing and working in a temporary construction work zone. The employer failed to ensure employees were trained in work zone safety standards, on-site specific conditions, procedures, traffic control plans and concepts.

29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: On or about October 18, 2022, employees working in the Westbound lane of Highway 34 on the Great River Bridge were exposed to struck-by hazards when setting up temporary traffic control barrels and the employer had not ensured the use of legible traffic signage at points of hazards to inform road users that road work was taking place and what actions the road users must take to drive through the work zone.

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices ("MUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA: On or about October 18, 2022, employees working in the Westbound lane of Highway 34 on the Great River Bridge were exposed to struck-by hazards when setting up temporary traffic control barrels and the employer had not ensured the use of temporary traffic control signs and arrow board for the protection of construction workers and to warn road users on HWY 34 and on the on-ramp that road work is taking place and what actions the road user must take to drive through the work zone.
