29 CFR 1910.106(f)(3)(iv)(c): bonding connections were not made to the vehicle or tank before dome covers are raised: a) In the rear parking lot at 1461 County Route 22, Gouverneur NY 13642, on or about 10/18/2022: An employee was tasked with transferring contaminated fuel, from a fuel delivery truck to an unbonded storage fuel truck, and was exposed to an explosion when gasoline vapors coming out the dome opening ignited.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE). If such hazards were present, or likely to be present, the employer did not: (i) Select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and (iii) Select PPE that properly fit each affected employee: (a) In the rear parking lot at 1461 County Route 22, Gouverneur NY 13642, on or about 10/18/2022: Glider Oil Company, Inc. had not performed a hazard assessment specific to the process of transferring contaminated fuel from one truck to another.
