29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: (a) SAMS West, INC, 9601 Grant St., Thornton, CO 80229: On or before October 20, 2022, an employee was operating a Hobart bandsaw in the meat department. The guard was up while the employee was prepping meat for cutting, exposing the employee's finger an amputation hazard.

29 CFR 1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) SAMS West, INC, 9601 Grant St., Thornton, CO 80229: On or before October 20, 2022, the employer did not effectively train employees on hazardous chemicals in the workplace. Employees did not know what a hazard communication program is or where to locate safety data sheets. This condition exposes employees to chemical hazards including, but not limited to exposure to: 1. Kayquat II (corrosive), and 2. Marketguard 121 (corrosive).
