29 CFR 1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about October 11, 2022 the employer did not ensure that the machine specific procedures of the plastic injection molding press ( E7) outlined the requirements for testing and ensuring the isolation of hydraulic pressure (hazardous energy) within the equipment. Employees are tasked with servicing, maintaining, and setting up of the injection molding machine as needed.

29 CFR 1910.147(f)(1)(ii):Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof; On or about October 11, 2022 the employer did not ensure that employees were removed from hazardous areas during positioning of the plastic injection molding press' ( E7) hydraulic cylinder ram. Employees are able to reach a pinch point where the ram nests into its attachment point (clevis) while manipulating it's controls from the operator station.
