29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  On or about October 20, 2022, at this worksite, employee engaged in roofing activities, utilized a ladder to access/egress the roof that did not extend at least three feet above the roof landing.
