29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: a) Soil Safe Inc.: On 10/5/2022, a Screener Operator fractured his right arm which resulted with time away from work. The employer did not record the injury and lost time of this employee on the OSHA 300 log or its equivalent within seven calendar days, on or about 10/20/2022. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Soil Safe Inc: On or about 10/5/2022, the employer did not ensure that procedures were documented for the control of potentially hazardous energy for the Screener Plant, when employees, such as, but not limited to, screen operators changed out screens, and cleaned the screener plant. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
