29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not fit tested prior to initial use of the respirator, were not fit tested whenever a different respirator facepiece (size, style, model, or make) was used, and were not annually fit tested: a) On 4 West and 5 West, on or about October 20, 2022 - Employees required to wear a tight-fitting filtering facepiece respirator were not fit tested prior to initial use of the respirator. Employees are required to wear a tight-fitting filtering facepiece respirator when working with patients or are in rooms with patients that are confirmed or suspected to have COVID-19. b) On 4 West, 5 West, and 6 West, on or about October 20, 2022 - Employees required to wear a tight-fitting filtering facepiece respirator were not fit tested with that respirator on an annual basis. Employees are required to wear a tight-fitting filtering facepiece respirator when working with patients or are in rooms with patients that are confirmed or suspected to have COVID-19.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a) On 5 West, on or about October 20, 2022 - A N-95 filtering facepiece respirator was worn by an EMS Housekeeping Aide who had facial hair that came between the sealing surface of the facepiece and the face. The EMS Housekeeping Aide is required to wear the N-95 filtering facepiece respirator when entering an isolation room with a COVID-19 positive patient.
