29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) On or about 10/20/2022, in the Weld Shop, employees did not utilize lockout procedures when doing maintenance, such as but not limited to, setting up/changing over the dies on the Mechanical Punch Press, exposing employees to amputation hazards if the Mechanical Punch Press is energized while employees were working on it.   b) On or about 10/20/2022, in the Weld Shop, lockout procedures were not developed for equipment with multiple sources of energy such as, but not limited to, the KOIKE Plasma Cutter which has pneumatic and electrical energy sources.   c) On or about 10/20/2022, in the Grinding Room, employees did not utilize lockout procedures when doing maintenance, such as but not limited to, replacing the blade on the HYDMECH S-20A Band Saw, exposing employees to amputation hazards if the Band Saw is energized while employees were working on it.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:  a) On or about 10/20/2022, Main Room, employees were exposed to flying debris and rotating parts hazards when operating lathes Clausing 17, Yamazaki 34, Yamazaki 24 and Leblond without guarding the chuck.   b) On or about 10/20/2022, in the Red Building, employees were exposed to flying debris and rotating parts hazards when operating the Clausing 13 lathe without guarding the chuck.   ABATEMENT DOCUMENTATION REQUIRED
