29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: (a) North side of steel building; Each employee working from the basket of a Snorkel aerial lift, model TB-47 JD7, did not stand on the basket floor, in that one of the workers in the basket stood atop the toeboard while installing a top rail piece. (b) North side of steel building; An employee working from a Skyjack vertical tower lift, model SJ16, did not stand on the basket floor, in that the worker exited the basket to perform work by sitting atop the mast of the vertical tower lift.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): (a) North side of steel building; A means of fall restraint or fall protection system was not worn while performing work from the platform of a Snorkel aerial lift, model TB-47 JD7. (b) North side of steel building; A means of fall restraint or fall protection system was not worn while performing work from outside the platform of a Skyjack vertical tower lift, model SJ16.
