29 CFR  1910.22(d)(2):Hazardous conditions on walking working surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired.  In the wash out room, employees were exposed to trip and fall hazards. Employees engaged in screen print washout activities, standing within 6 inches, of a sump pump hole measuring approximately 18 inches across and more than 18 inches in depth.

Employees engaged in screen print cleaning activities were exposed to corrosive chemical hazards. The employer had not assessed the workplace to determine the need for personal protective equipment. Employees utilize corrosive ink remover, REMOVE IR26, without determining the need for personal protective equipment. The REMOVE IR26 GHS label states "Wear protective gloves/protective clothing/eye protection/face protection."

The employer did not have an eyewash station present in the workplace where corrosive chemicals, including but not limited to ink remover, used by employees to clean screen printing screens.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  In the washout room, employees engaged in screen print washout activities were exposed to corrosive health hazards. The employer did not label secondary containers of cleaning chemicals for removing inks.
