29 CFR  1910.37(a)(3): Exit route(s) are not kept free and unobstructed:  (a)	Stock Room: On or about 10/20/2022, the exit route to the white exit door, adjacent to the roll up delivery loading door, is not kept free and unobstructed in that it is completely blocked by the storage of large piles of merchandise of excessive heights. These piles are covering the full width of this section in the stock room. These piles consist of various merchandise containing boxes, U-boats full of merchandise boxes, and flats of peanut butter and other flats of canned goods, soda and drinks.  (b)	Rear Storage Area Adjacent to the Front Wall: On or about 10/20/2022, the marked exit route from the stock room to the back end of the store is not kept free and unobstructed in that it is completely blocked by the permanent set of shelving which is actively storing merchandise. These blocking items consist of a large pile of excessive heights of boxes full of various merchandise, U-boats full of merchandise and flats of soda and other heavy food goods.  The employer routinely fails to ensure that exit routes are kept free and unobstructed as required by the standard, as demonstrated on October 20, 2022, when exit routes were blocked and/or reduced in width in the Storeroom of the Dollar Tree store at 342 Power Road, Pawtucket, RI 02860.  Dollar Tree Stores, Inc. was previously cited for a Repeat - Serious violation of this occupational safety and health standard, 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 1464979, citation number 2, item number 1 and was affirmed as a final order on 04-05-2021, with respect to a workplace located at 999 North Montello Street, Brockton MA 02301. 29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  (a) Stock Room Main Exit Door:  On or about 10/20/2022, portable fire extinguishers were not located so that they were readily accessible without subjecting employees to injuries in that they were completely blocked by the storage of boxes of merchandise, piled up to approximately 6 feet 11 inches in height.  (b) Stock room wall adjacent next to the second exit: On or about 10/20/2022, portable fire extinguishers were not located so that they were readily accessible without subjecting employees to injuries in that they were completely blocked by the storage of boxes of merchandise, piled up along the back wall. Employees were incapable of reaching this space due to the amount of boxes stacked in front of the wall where only the sticker can be seen. 29 CFR  1910.176(b):Material stored in tiers are not stacked, blocked, interlocked or limited in height so that it is stable and secure against sliding and collapse:  (a) Throughout the Stock Room: On or about 10/20/2022, the employer is not ensuring that materials such as, but not limited to, boxes both full of merchandise and empty, flats of soda, canned goods, other heavy grocery items and cases of cleaning supplies are stacked securely. These boxes of merchandise and flats are not stacked in a secure manner nor stored in a blocked pattern and are stacked up to excessive heights, exposing employees to struck-by hazards.  The employer routinely fails to ensure that material is stored securely as required by the standard, as demonstrated on October 20, 2022, when boxes of merchandise were not stacked securely in the Storerooms of the Dollar Tree store at 342 Power Road, Pawtucket, RI 02860.
