29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a. Second Floor: An outlet box designed to be mounted in a wall was attached to the end of an extension cord to power a water cooler and power tools. Employees were exposed to fire and electrical hazards.  b. Second Floor: Three extension cords were connected in series "daisy chained" providing power to a water cooler and power tools. Employees were exposed to fire and electrical hazards.
