29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about October 14, 2022, the employer did not ensure that heading operator employees received authorized employee lockout tagout training thereby exposing them to struck-by and crushing hazards when removing the machine guard to make adjustments within the tooling area of Header #21, 3/16 National Single Die Tubular Rivet Header, SN36998. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about October 14, 2022, the employer did not ensure that a heading operator employee implemented energy control application steps prior to allowing them to make adjustments within the tooling area of Header #21, 3/16 National Single Die Tubular Rivet Header, SN36998. The machine was not shut down or turned off prior to the employee engaging in this servicing activity, thereby exposing them to struck-by and crushing hazards from the unexpected movement of the machine. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
