29 CFR  1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about October 18, 2022, employees conducting masonry installation operations from as supported frame scaffold on a residential single-family property were exposed to fall hazards of at or above 25' when the employer failed to ensure a ladder was provided to ascend and descend from the ground to the upper level.   No abatement certification or documentation is required for this item.

29 CFR  1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On or about October 18, 2022, an employee working from a boom lift on masonry installation for a residential single-family property was exposed to fall hazards of at or above 25' when the employer did not ensure a personal fall arrest system was utilized.   No abatement certification or documentation is required for this item.

29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  On or about October 18, 2022, employees conducting masonry installation operations from as supported frame scaffold on a residential single-family property were exposed to fall hazards of at or above 25' when the employer failed to ensure that the supported frame scaffold was fully planked at the working level.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1530248, citation number 1, item number 2(A) and was affirmed as a final order on July 13, 2021, with respect to a workplace located at 1 S. 415 Luther Ave., Villa Park, Illinois.  No abatement certification or documentation is required for this item.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  On or about October 18, 2022, employees conducting masonry installation operations from as supported frame scaffold on a residential single-family property were exposed to fall hazards at or above 25' when the employer failed to ensure that fall protection was utilized.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1530248, citation number 1, item number 3 and was affirmed as a final order on July 13, 2021, with respect to a workplace located at 1 S. 415 Luther Ave., Villa Park, Illinois.  No abatement certification or documentation is required for this item.
