29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  NEW RADIOLOGY BASEMENT MECHANICAL ROOM: On October 14, 2022, during the replacement of a steam compensator, a lockout or tagout device was not affixed to the blow down bleed valve until the maintenance was completed, exposing employees to thermal energy hazards.

29 CFR  1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished:  BUILDING 107 CRAWLSPACE: On October 14th, 2022, an authorized employee did not verify the isolation of the first OS&Y shutoff valve before beginning maintenance on a main steam line compensator, exposing employees to hazardous thermal energy.

29 CFR  1910.147(c)(4)(i): NAVAHCS failed to develop, document, and ensure employees used procedures for the control of potentially hazardous energy when required.  MANHOLE VAULT 20: On October 14, 2022, employees replaced a steam compensator without documented procedures for controlling the hazardous energy of the main steam line. Employees implemented unsafe procedures that allowed for the potential to release hazardous energy sources such as thermal, hydraulic, kinetic, and pneumatic.

29 CFR  1910.147(c)(7)(i)(A): NAVAHCS failed to ensure that authorized employee(s) received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.   MAINTENANCE AND OPERATIONS DEPARTMENT: On October 14, 2022, NAVAHCS failed to provide proper training for specific equipment procedures that included the method and means necessary for energy isolation and control to all authorized employees who performed maintenance on a new steam compensator in Manhole Vault 20.  OSHA previously issued a Notice to the U.S. Department of Veterans Affairs for a violation of this occupational safety and health standard in inspection #1502795, with respect to a workplace in West Haven, Connecticut, which became a final order on June 10, 2021.

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a) BUILDING 107 CRAWL SPACE: On October 14, 2022, each authorized employee failed to affix a personal lockout or tagout device to a group lockout device on the first steam line valve before replacing a steam compensator in Manhole Vault 20.  b) MANHOLE VAULT 20: On October 14, 2022, each authorized employee failed to affix a personal lockout or tagout device to a group lockout device on the second OS&Y isolation valve before replacing a steam compensator in Manhole Vault 20.   OSHA previously issued a Notice to the U.S. Department of Veterans Affairs for a violation of this occupational safety and health standard in inspection #1502795, with respect to a workplace in West Haven, Connecticut, which became a final order on June 10, 2021.
