At the Northeast side of the Duffey SE TD Bank commercial project located at/around 215 Harper Lane, Saint Johns, Florida, 32259: On or about October 21, 2022, the employer exposed employees to fall hazards ranging from 6 to 12-feet, in that the employees were using the top step of a 10-foot Werner ladder to access and/or to install aluminum panels on an awning 12 feet high. b. At the Northwest side of the Duffey SE TD Bank commercial project located at/around 215 Harper Lane, Saint Johns, Florida, 32259: On or about October 21, 2022, the employer exposed employees to fall hazards ranging from 6 to 12-feet, in that the employees were using the top step of a 10-foot Werner ladder to access and/or to install aluminum panels on an awning 12 feet high.

At the Duffey SE TD Bank commercial project located at/around 215 Harper Lane, Saint Johns, Florida, 32259: On or about October 21, 2022, the employer exposed employees to fall hazards ranging from 6 to 12-feet, in that the employees were standing, sitting, and/or walking on an anchored aluminum awning frame twelve feet high without fall protection. The Florida Commercial Awning, Inc. DBA The Awning Factory, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 1382246, citation number 1, item number 1, and was affirmed as a final order on 4/16/2019, with respect to a workplace located at 128985 Livingston Rd, Naples, Florida 34105.
