29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a. At the jobsite located at coordinates 28.75524 "N 81.98782 "W, Sumterville, Florida: On or about October 20, 2022, the employer did not instruct compact roller operators in the hazards associated with operating rollers in reverse over uneven soil, the procedures to follow during a tipping event, and the specifics of the operation of the equipment and safety precautions included in the manufacturers manual, exposing employees to a crushed-by hazard.
