29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job site, materials and equipment to be made by competent persons designated by the employer: a. AVI, Inc. dba AVI Roofing & Gutters, Inc., On October 12, 2022 as the exposing employer, did not protect its employees from slips and trip hazards and an employee was directed to work on the roof of new residential construction when the ladder being used slipped causing the employee to fall over 15 feet. The employer failed to detect and prevent changing work conditions to protect employees. This condition exposed employees to broken bones and head injuries

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. AVI, Inc. dba AVI Roofing & Gutters, Inc., On October 12, 2022 the exposing employer did not ensure that employees were protected from injury in that employees were conducting roofing operations at heights over 15 feet on new residential construction without the use of fall protection. This condition exposes to employees to head injuries and broken bones. AVI, Inc. dba AVI Roofing & Gutters, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1478602, citation number 1, item number 1 and was affirmed as a final order on August 10, 2020, with respect to a workplace located at 9151 E Oxford Ave. Denver, CO 80237.
