29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. U.S. Department of Veterans Affairs -Northern California Health Care System, Mather - The employer did not perform a Personal Protective Equipment (PPE) assessment in writing for employees in the SPS department that use the Medivator and Rapicide Parts A and B. Employees were exposed to chemical and bloodborne pathogen hazards. The employer was previously cited for a violation of this occupational safety and health standard certified in OSHA inspection number 1359370, citation 2, item 1 and was affirmed a final order on February 13, 2019, with respect to a workplace location at U.S. Department of Veterans Affairs -Northern California Health Care System, Mather.
