29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) On or about October 25, at 2550 30th Ave N, St. Petersburg Beach, FL, 33606,  the employer did not ensure that employees operating the forklift to move metal gaylord boxes, load and unload metal scraps, were trained prior to operating a powered industrial truck , exposing employees to roll-over, crushed-by, fall and struck-by hazards.

29 CFR  1910.303(g)(1)(vi)(B): A three foot minimum clearance was not maintained for headroom of working spaces about service equipment, switchboards, panel boards, or motor control centers.   a) On or about October 25, 2022, in the warehouse, employees were exposed to an electrical contact hazard, in that, the electrical panel was blocked by stored palletized items which prevented the 3-foot minimum clearance from being maintained.
