29 CFR 1910.1200(g)(10): The employer did not ensure that, in all cases, the required information was provided for each hazardous chemical, and was readily accessible during each work shift to employees when they were in their work area(s): a) Amazon.com Services LLC - PHL7 at 560 Merrimac Avenue in Middletown, DE 19709: On or about October 20, 2022 hazardous chemicals were applied by a contractor when no safety data sheet or other chemical information was obtained by Amazon.com Services LLC - PHL7, which resulted in delayed medical treatment when an employee was required to work in the AFE Re-bin area in proximity to the application process for several hours and required treatment for the symptoms they were having.

Violative instances included but were not limited to: a) Case No. 108 (Mods) occurring on 10/14/2022 did not contain a complete entry in Column F, description for the recordable injury. Column F only contained 'AA was performing daily pick tasks'. Failing to describe the illness or injury, struck by a box that fell from upper level bin; parts of the body affected, hand/wrist; and object/substance that directly injured or made person ill, box of trash bags. b) Case No. 109 (Mezz small singles pack area) occurring on 10/12/2022 did not contain a complete entry in Column F, description for the recordable injury. Column F only contained 'AA was performing daily tasks in pack, single smalls'. Failing to describe the illness or injury, hand pain; parts of the body affected, right hand; and object/substance that directly injured or made person ill, using strong C grip while performing daily tasks.

Violative instances included but were not limited to: a) Case No. 113 (AFE - breathing difficulty/allergic reaction) occurring on 10/20/2022 had an OSHA Form 301 that was not filled out in a complete manner. Question 6 was not filled out and failed name the physician or other health care professional. Question 7 was not filled out and failed to list the location treatment was given away from the worksite. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
