29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 1, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations. (b) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 2, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations. (c) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 3, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations. (d) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 4, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations. (e) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 5, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations. (f) On or about October 25, 2022, employees were exposed to a caught in/nip point hazard at the in-feed section of the Glebar GT-9AC grinders 6, in-that the shuttle at the rest position is unguarded when the shuttle starts the slide operation and finishes the slide operations.
